Health Department works to make sure its services are available to
all people. We offer certain no fee services, such as child
immunizations and WIC. We also provide a sliding fee scale (reduced
fees) for quality health care services, based on certain factors. To

We offer health care
services that are available to EVERYONE. These services have no
eligibility requirements, although some require financial screenings
and/or fees for services rendered. These services include, but are
not limited to:

only to persons who meet our eligibility requirements. These
services include:To determine if you qualify, you must first schedule an
eligibility appointment with the DeSoto County Health Department
by calling (863) 993-4601. Before we can determine if you are
eligible, you must have the proper documents with you at your
first visit. This is what you’ll need:
- Government-issued Photo ID
- Social Security Cards for EVERYONE in your household.
- Proof that your are a current DeSoto County resident (such as a
recent utility bill or rental agreement)
- Proof of income for EVERYONE in your household (if
applicable)
Remember—each person’s eligibility status is unique. At your
initial visit, we may require you to provide further
